Day Shift 5 x 8s, M - F 8am to 5pm. Must have LVN certification. At least 1 year of experience. This will be a clinical intake coordinator for home health processing and scheduling referrals. No Call. A possible job title for this role could be LVN Clinical Intake Coordinator. Candidates must have their LVN.

Responsibilities
Coordinate intake of home health referrals and ensure timely processing.
Prioritize referrals, manage pipelines, and forecast potential admissions.
Ensure compliance with HIPAA, policy, and regulatory requirements.
Verify patient eligibility, benefits, and admission criteria with accuracy.
Maintain accurate patient records in the EHR system and ensure data integrity.
Communicate intake status and next steps with families, clinicians, and payers.
Provide accurate KPI reporting on intake volumes, conversion rates, and timelines.
Escalate high-risk cases to appropriate clinicians and leadership.
Collaborate with operations to schedule staffing and resource allocation.
Identify opportunities for process improvements to reduce delays.
Support on-site team with training and onboarding of new intake staff.
Experience/Qualifications
Strong knowledge of HIPAA, patient confidentiality, and regulatory compliance.
Experience in intake coordination for home health or similar healthcare settings.
Proficiency with EHR systems and data integrity best practices.
Experience forecasting admissions and managing intake pipelines with KPIs.
Ability to verify patient eligibility and benefits accurately for admissions.
Excellent communication with families, clinicians, and payers; escalation protocols.
Strong organizational skills with prioritization and scheduling for staffing.
Process improvement mindset; identify delays and implement solutions.
Capability to generate and analyze intake performance reports and dashboards.
On-site training and onboarding support for new intake staff.
