Intake Coordinator / Licensed Vocational Nurse (LVN)

About the Role:
Blize Healthcare is seeking a detail-oriented and compassionate Intake Coordinator / Licensed Vocational Nurse (LVN) to join our team. This hybrid role combines office-based intake coordination with in-home patient care, ensuring a seamless transition for patients entering home health and hospice services.
This position requires both administrative work in an office setting and travel to patient homes to provide in-person nursing care.
Key Responsibilities:
Intake Coordinator Duties: (Office-Based)
- Manage daily patient referrals and intake operations.
- Verify insurance coverage and assist with claims processing.
- Schedule home health and hospice visits efficiently.
- Answer, screen, and direct phone calls.
- Respond to patient and family inquiries in a professional and timely manner.
- Maintain medical supply inventory and prepare DME (Durable Medical Equipment)
- Coordinate patient admissions to ensure a smooth onboarding process.
LVN Duties: (Field-Based)
- Travel to patient homes to provide skilled nursing care.
- Perform basic nursing procedures (medication administration, wound care, vital signs)
- Support patients under the direction of a Registered Nurse or physician.
- Build compassionate relationships with patients and their families.
- Monitor patient conditions and report changes appropriately.
Qualifications:
- Minimum 1 year of experience in the medical field.
- Minimum 1 year of nursing experience.
- Home health or hospice experience preferred.
- Strong communication, interpersonal, and customer service skills.
- Ability to manage both administrative and clinical responsibilities.
- Proficiency in Microsoft Office and Google Suite.
Requirements:
- Active LVN license.
- Valid BLS certification.
- Valid driver’s license and reliable transportation.
- Ability and willingness to travel to patient homes for in-home visits is required.
