About the Role:
Family Paramount Hospice LLC is looking for a compassionate and organized Intake Coordinator to join our dedicated team in Naperville, IL. In this vital role, you'll be the first point of contact for patients and families navigating one of life's most sensitive transitions, ensuring a smooth and supportive admissions process. Responsibilities:

• Manage the hospice intake process from initial referral through patient admission
• Coordinate with physicians, hospitals, skilled nursing facilities, and care teams to gather clinical information
• Communicate compassionately with patients, families, and caregivers to explain hospice services and eligibility
• Verify insurance benefits, including Medicare, Medicaid, and private insurance for hospice coverage
• Maintain accurate and timely documentation within the electronic medical records (EMR) system
• Collaborate with clinical and administrative staff to ensure seamless care transitions
• Track and follow up on pending referrals to maximize timely patient enrollment

Requirements:

• Previous experience in hospice, home health, or healthcare intake/admissions strongly preferred
• Familiarity with hospice regulations, Medicare/Medicaid eligibility, and insurance verification
• Excellent communication and interpersonal skills with a compassionate, patient-centered approach
• Strong organizational skills with the ability to manage multiple referrals simultaneously
• Proficiency with EMR systems and general computer applications
• Ability to work effectively in a fast-paced, emotionally sensitive environment
• High School Diploma or equivalent
