Job Summary

The Intake Coordinator is responsible for managing the initial intake process for patients, ensuring accurate documentation, and coordinating services to support smooth transitions of care. This role acts as a liaison between patients, families, and healthcare providers, facilitating communication and ensuring timely access to care. The Intake Coordinator supports various departmental needs and may specialize in areas such as care coordination or patient navigation, depending on departmental requirements.

Essential Functions

  • Coordinates the intake process by collecting and verifying patient information, medical history, and insurance details.
  • Serves as the primary point of contact for patients and families during the initial stages of care, providing guidance and addressing questions or concerns.
  • Facilitates communication between patients, providers, and other departments to ensure seamless transitions and continuity of care.
  • Maintains accurate and up-to-date documentation in electronic medical records (EMRs) or other systems, ensuring compliance with regulatory and organizational standards.
  • Schedules appointments, tests, or services as needed, ensuring alignment with patient needs and departmental workflows.
  • Collaborates with multidisciplinary teams to address barriers to care and improve patient access to services.
  • Supports quality improvement initiatives by identifying opportunities to streamline intake processes and enhance patient satisfaction.
  • Provides education and resources to patients and families to support their understanding of care plans and available services.
  • Monitors and tracks referrals, authorizations, and other intake-related metrics, ensuring timely completion and follow-up.
  • Performs other duties as assigned.
  • Maintains regular and reliable attendance.
  • Complies with all policies and standards.

Qualifications

  • Associate Degree in a healthcare related field preferred
  • 0-2 years of experience in a healthcare or patient-facing role, with prior intake or care coordination experience required

Knowledge, Skills and Abilities

  • Knowledge of medical terminology, healthcare systems, and patient intake processes.
  • Strong organizational skills with attention to detail and accuracy in documentation.
  • Excellent communication and interpersonal skills for interacting with patients, families, and multidisciplinary teams.
  • Proficiency in electronic medical record (EMR) systems and other healthcare software.
  • Ability to prioritize tasks, manage multiple responsibilities, and adapt to dynamic workflows.
