Our client is a well-established and growing home health agency dedicated to delivering compassionate, patient-centered care throughout the community. They are seeking an experienced Intake Coordinator to join their team in Sarasota. This is a full-time, in-office position that plays a critical role in ensuring patients receive timely, high-quality home health services.

The ideal candidate has previous home health intake experience, understands the referral and admission process, and is knowledgeable about insurance verification and payer requirements. This role requires someone who is highly organized, detail-oriented, and able to thrive in a fast-paced healthcare environment while providing exceptional service to patients, families, referral sources, and clinical staff.

Primary Responsibilities

  • Receive, review, and process incoming home health referrals

  • Verify insurance eligibility, benefits, and authorization requirements

  • Coordinate new patient admissions with clinical, scheduling, and operations teams

  • Communicate with physicians, hospitals, case managers, discharge planners, and referral partners regarding patient referrals and start-of-care timelines

  • Ensure all intake documentation is complete, accurate, and compliant with Medicare, Medicaid, commercial insurance, and company requirements

  • Enter and maintain patient information within the electronic medical record (EMR) system

  • Monitor referral status and proactively resolve issues that could delay patient admissions

  • Answer incoming calls and provide professional, compassionate customer service

  • Maintain patient confidentiality and comply with HIPAA regulations

  • Assist with additional administrative and intake responsibilities as needed

Qualifications

  • Previous home health intake experience strongly preferred

  • Knowledge of Medicare, Medicaid, commercial insurance, and authorization processes

  • Experience verifying insurance benefits and eligibility

  • Proficiency with electronic medical record (EMR) systems

  • Excellent organizational skills with exceptional attention to detail

  • Ability to prioritize multiple tasks in a fast-paced environment

  • Strong verbal and written communication skills

  • Professional, dependable, and collaborative work style

Compensation & Benefits

  • $23.00 - $27.00 per hour
