This position will work 830-5 or 9-530, with weekend and holiday rotation.The Intake Nurse is integral in the coordination and execution of the admission process to Hospice services. Demonstrates excellent customer service skills with patients, families, customers, and co-workers. Must be resourceful in overcoming hospice barriers with patients and caregivers. Responsible for obtaining referral orders and patient records.Unrestricted LPN or RN license. Minimum of 2 years of nursing experience in home health, or healthcare facility.. Must be able to deal with human suffering while maintaining professional boundaries. Required to be flexible and change schedule at a moment's notice to meet patient needs.
