Pay: $32-$35 Hourly Schedule: Monday - Friday 4pm-12am Responsibilities
The Intake LPN works closely with the Director of Nursing (DON) and nursing team to coordinate patient admissions, perform clinical assessments, and ensure a safe, high-quality care environment. This role combines clinical expertise with patient-focused coordination to support effective intake processes and continuity of care. Conduct initial nursing assessments to determine patient appropriateness for admission Oversee and support patient admissions, transfers, and discharges Collaborate with nurses to monitor, assess, and document patient care during intake Assist nurses in communicating treatment plans and liaising with providers Identify areas of non-compliance and provide in-services as needed Complete intake-related audits and ensure daily checklists are completed accurately and on time Assist with shift handoffs by providing clear patient information to the nursing team Identify high-risk patients and communicate with the provider regarding treatment plans Ensure correct documentation and follow procedures for patients requiring higher levels of care (HLOC) Coordinate return to facility for patients sent to HLOC as necessary Administer medications safely and efficiently Conduct random narcotic counts and follow medication handling protocols Monitor medication availability and follow up with pharmacy as needed Assist in conducting weekly urine drug screens Receive, analyze, and upload lab results, communicating findings to providers Complete administrative duties as assigned by the DON Perform additional tasks as needed to support patient care and intake operations
Qualifications Licensed Practical Nurse (LPN) in good standing in the state of practice Valid BLS certification required Associate degree in nursing or equivalent education preferred Minimum 2 years of nursing experience in a behavioral health, mental health, or substance use treatment setting preferred Experience in chemical dependency/addiction treatment preferred Personal passion or interest in substance use recovery strongly preferred Commitment to a drug-free lifestyle and responsible alcohol use Leaders in active recovery must possess a minimum of 2 years of sobriety
