Intake Nurse LVN
Completes functions related to the intake process for Home Health patients for assigned team including workflow within the Electronic Medical Records (EMR) system. Responsibilities include, but are not limited to, managing the intake process for Home Health, setting up patient account information, and facilitating face-to-face physician encounter visit.
What you will do in this role:
Reviews, approves and coordinates all referrals directed toward assigned care team. Manages all assigned intake/admission EMR workflow for Home Health patients (initial referrals, physician verification, etc.) per assigned care team.
Provides daily reporting to care team regarding pending admissions, new admission and outstanding (open) items.
Obtains initial payer source eligibility verification upon intake/referral and coordinates indicated follow-up necessary to adequately communicate the patient’s expected out-of-pocket expense for services.
Coordinates with care team in facilitating completion of physician face-to-face patient appointment/encounter visit. Responsible for the face-to-face encounter workflow process as assigned within the EMR, which includes contacting the physician’s office to verify visit or schedule visit, request visit documentation, and log activity within the EMR.
Obtains verbal orders, within scope of practice, as needed.
What qualifications you will need:
Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
Minimum of two years’ experience in an office setting, one year of home health agency experience is preferred
Must have ability to obtain licensure in additional states
Proficiency with computer use and software applications including MS Office software products required and experience with EMR software preferred
Current BCLS prior to delivering patient care
Reliable transportation and proof of valid automobile liability insurance
Must have valid driver’s license
