JOB DESCRIPTION
INTAKE/SCHEDULER
Responsible for coordinating, planning and documenting patient care staff and patient schedules in HCHB. Performs HCHB functions, in order to improve the clinical, operational and financial success of the agency.
QUALIFICATIONS
- An LPN or RN currently licensed by the state in which employed by the agency.
- At least one year of experience in home health care.
- Requires excellent interpersonal skills and communication, as well as the ability to effectively interact with all levels of management.
- Must have strong organizations skills and be able to effectively manage and prioritize tasks and be time efficient.
- Computer experience necessary to perform job responsibilities in the HCHB process.
- Good working knowledge of the patient Referral process.
KEY RESPONSIBILITIES
- Receives/responds to incoming telephone calls from referral sources/patients. Obtain patient orders, in order to process the Referral.
- Establish patient and employee schedules by coordination of patient’s needs and staff availability. Process schedules and re-schedules, re-assignments and declined visits.
- Input all essential patient and employee schedule information in HCHB in an accurate and timely manner. Complete all requests according to priority and necessity of the patient’s needs.
- Coordinate schedule of patient and relays it to the appropriate party, so that optimal patient care will be provided. Relay to the RN Team Leader cases that have a delayed start and reason for the delay.
- Maintain a current and accurate record of direct nursing field staff availability and coordinates assignment of nursing personnel according to specific patient needs and requests. Assign the Referral to the staff employee in HCHB for the Admission process.
- Communicate all problems and concerns of patient services and staff employee performance/issues to the DOPS.
- Reschedule staff employee schedules according to illnesses, emergencies, etc. Inform all clinical staff of any change in scheduling.
- Produce the following reports:
- Staff employee Schedule Report;
- Visits request Report;
- Staff employee calendars;
- Unverified visits Report.
- View the staff employee Synch Summary to ensure all synchronizations have the completion feature required.
- Reviews/Audits nursing visit report, supplemental orders, home health aide visit report and special discipline notes on 10% of Active & Discharged patient charts each Quarter.
- May attend PI Utilization Review Committee on a quarterly basis or as scheduled.
- Organizes, prioritizes and executes work assignments effectively and efficiently.
- Receives instructions and keeps ongoing contact with Team Leaders and Director of Patient Services in assigned offices.
- Attends case conferences, inservices, seminars as scheduled, and other meetings as assigned by Director.
- Makes patient visits if requested.
- Maintains confidentiality of information related to business practices, business activities, and personnel.
- Maintains a professional attitude when working with staff, fostering the team philosophy and team environment for all personnel.
- Does not allow personal affairs to interfere with scheduled work time.
- Maintains a professional appearance at all times.
- Provides proper notification and/or advanced notice of absence or tardiness without abuse.
- Participates willingly with special projects and/or overtime work when requested.
- Follows all agency policy and procedures.
