    Intake Service Coordinator
     
    Program Overview 
     
    Support Services for Veteran Families (SSVF) serves veterans and their families in need of support; we have teams located in our corporate office, as well as our housing developments in Humboldt Park and Englewood. Our Intake Service Coordinators serve as the primary points of contact for providing short-term transition support to newly enrolled participants in the SSVF program. Our goal is to provide clients with a clear, coordinated pathway into VOA Illinois services within the first 90 days, as well as ensuring they are successfully connected and transitioned into the programs best met for their needs. 
     
    Position Summary 
     
    The Intake Service Coordinator role serves as the primary resource for client access, program eligibility, intake, enrollment, and service navigation. Working collaboratively with internal programs and community partners, the position facilitates timely access to services, coordinates seamless transitions into ongoing case management, and promotes continuity of care by connecting clients to appropriate internal and external resources. 
     
          Primary Responsibilities
     
          Client Engagement & Enrollment 
    • Serve as the primary point of contact for potential clients seeking services 
    • Conduct intake assessments and eligibility screenings 
    • Assist with service planning, resource coordination, and stabilization efforts 
    • Educate clients on available programs and services
          Data & Document Management 
    • Accurately document client sessions in EchoVantage (EV) and Homeless Management Information System (HMIS) 
    • Ensure all documentation falls within HIPAA regulations as well as VOA guidelines 
          Collaboration & Advocacy 
    • Utilize a trauma-informed approach in all client interactions 
    • Partner with internal VOA Illinois teams and community partners for service coordination 
    • Advocate for participant needs and reduce barriers to service access 
    Minimum Qualifications 
     
           Education
    • Must meet one of the minimum requirements as a Mental Health Professional (MHP) in the State of Illinois: 
      • Bachelor’s degree 
      • Licensed Practical Nurse (LPN) 
      • Certified Psychiatric Rehabilitation Practitioner, Recovery Support Specialist, or Occupational Therapy Assistant meeting required experience standards; or 5 years of supervised mental health or human services experience (including qualifying internships) 
            Experience
    • Experience in social services, crisis intervention, substance abuse/mental health issues, and familiarity with community resources is essential 
    • Experience supporting veterans or lived experience as a veteran preferred but not required 
            Location
    • Downtown corporate office located at 47 W. Polk St 
            Schedule
    • Monday-Friday, 9am-5pm 
     
    Compensation & Benefits 
     
    The Intake Service Coordinator is a full-time, non-exempt role: 
    • Pay Range: $19.30 - $26.64/hour
