What You’ll Do
Coordinate and process incoming home health referrals
Gather and verify patient, insurance, and referral information
Enter accurate referral information into the electronic medical record (EMR)
Communicate with hospitals, physicians, facilities, insurance case managers, patients, and team members
Help coordinate appropriate start-of-care dates
Follow up on incomplete referrals and help keep the intake process moving
Manage incoming fax and website communications

High school diploma or equivalent
Medical terminology coursework or equivalent education/experience
Medical office experience required
Nursing background (CNA, MA, Med Tech) welcomed, but not required
