Objective/Overview
In pursuit of its mission, Transforming care and education to advance the health of our patients and the communities we serve, CUHCC provides medical, dental, and behavioral health care to nearly 12,000 racially and culturally diverse patients annually. The vast majority of patients live below federal poverty lines and more than half require language interpretation services.
The clinic integrates primary care, behavioral health, and dental services. Our behavioral health department encompasses outpatient psychiatry and therapy, Substance Use Disorder services, and IBH-Care Coordination embedded in primary care. Community programs, including ARMHS and Targeted Case Management, further support whole-person care. A key aspect of this role is to facilitate collaboration across these service lines and programs, promoting team-based care as CUHCC continues to fully implement the Health Care Home model.
The Integrated Care & Community Health Program Manager provides dual clinical and programmatic leadership for CUHCC's Integrated Behavioral Health-Care Coordination (IBH-CC) team and the Community Health Team, composed of community health workers (CHWs). This role combines direct patient care with operational oversight and clinical supervision. The Program Manager ensures high-quality, trauma-informed, culturally grounded behavioral health and community health services within a highly diverse primary care clinic.
Reporting Relationships
This position reports to the Mental Health Director and directly supervises the following staff: 1.0 FTE IBH licensed clinician, 2.0 FTE care coordinators, 2.5 FTE Community Health Workers and clinical trainees in practicum.

At CUHCC, were transforming urban health care and educating the next generation of healthcare professionals.
CUHCC offers a full range of dental and mental health services that are typically not offered at a community health center, such as prosthodontic services and psychiatry. CUHCC has pharmacists, psychiatric nurse practitioners, dentists, dental hygienists, nurses, family nurse practitioners, family practice physicians, internal medicine physicians, internal medicine-pediatric physicians, licensed social workers and psychologists among other disciplines on staff.
CUHCCs interprofessional care model uses a collaborative, team-based approach to deliver education and patient care, integrating these services. Students and residents at CUHCC have the opportunity to work with other students and preceptors in other fields to best serve the complex needs of the patients. Over half of CUHCC's mental health patients also receive medical services at CUHCC, allowing CUHCC providers to work collaboratively to provide patient centered care.
As a founding member in the countrys first accountable care organization (ACO) for federally qualified health centers, CUHCC is leading change in urban health and improving patient outcomes to meet the Institute for Healthcare Improvements Triple Aim: improve the patient experience, improve patient health, and reduce the cost of patient care.
Pay and Benefits
Pay Range: Between $86,000 - $93,000/annually for 1.0 FTE, depending on qualifications. Salary range is based on experience and overall mastery/job proficiency level of position.
