    Job Summary

    Responsible for implementing workflow education and training for the care management staff throughout the Prisma Health Integrated Care Management department. Workflow training may include new employee orientation, ongoing staff development training that is in compliance with internal and external guidelines as well as standard current practice. Escalates team needs for technology systems, reports and analytics, and the care management teams to ensure appropriate training is developed to support the development of workflows, care pathways and guidelines to manage a large patient population across the Prisma Health footprint.

    Job Description

    Essential Functions

    • Leads training for Integrated Care Management IT technologies. Works extensively with the care management team and leaders to ensure workflow is comprehensive and supports the patient populations they serve.

    • Remains current and knowledgeable about care management and population health training, current practices, documentation, and reporting in the healthcare industry. Works to drive innovation into the integrated care management education and training programs.

    • Assists in the development of new processes, pathways, guidelines, and workflows for use by the interdisciplinary care management team

    • Performs ongoing audits to ensure compliance with workflows and maintains operational dashboards for process improvement efforts. Coordinates with leaders to help identify targets and coordinate efforts to achieve said targets.

    • Conducts new employee orientation and ongoing staff development that is in compliance with national quality standards and standard current practice.

    • Serves as program liaison to communicate across multiple teams with key stakeholders to identify needs for new value-based programs, refine existing programs, and conclude idle programs using a population health lens and in alignment with enterprise-level strategic initiatives, CMS regulations and/or NCQA accreditation.

    • Performs other duties as assigned.

    Supervisory/Management Responsibilities

    • This is a non-management job that will report to a supervisor, manager, director, or executive.

    Minimum Requirements

    • Education - Bachelor's degree in Nursing/Health Informatics or Master's degree in Social Work.

    • Experience - Two (2) years inpatient nursing, care or case management experience, education, training, workflow development experience with healthcare/care coordination technology systems and reporting

    Required Certifications, Registrations, Licenses

    • LMSW (Licensed Master Social Worker) or LISW (Licensed Independent Social Worker) in South Carolina or holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.

    Knowledge, Skills and Abilities

    • Capable of and experience in clearly conveying learning objectives and creating a positive learning experience.

    • Communication techniques to drive positive learning experiences

    • Computer skills (word processing, spreadsheets, databases, data entry)

    • Knowledge of office equipment

    • Knowledge of how to conduct learning needs assessments, evaluate learning outcomes and monitor training costs preferred
