A Brief Overview

The Integrated Care Manager is responsible for providing assistance to individuals with housing insecurity. The coordinator helps clients secure stable housing, assists with housing applications, and works with various stakeholders to support housing initiatives. This position also helps individuals overcome barriers to health such as vital documents, other social determinants of health barriers and establishing and holding individuals accountable to an integrated care plan that includes (though not limited to) medical care access, behavioral health care access, and

addiction treatment access.

Qualifications

Education

  • Bachelor's Degree (Required) or
  • Combination of relevant education and experience may be considered in lieu of degree (Required)

Experience

  • 1 year experience in housing services, social work, or community outreach (Required)
  • 2 years experience with the Philadelphia Homeless Shelter System (Required)
  • General Experience and working knowledge of mental health and substance use disorders (Required)
  • General Experience and comprehensive knowledge of the various resources within the City of Philadelphia (i.e. Office of Homeless Services, DBHIDS, etc) (Required)
