General Summary:

An Intensive Case Manager (ICM) at Safer Foundation is responsible for determining potential clients' eligibility/suitability for services, conduct the enrollment process, and provide case management services during program enrollment and follow-up. These services include but are not limited to supporting program recruitment efforts, conducting assessments, assisting with client barriers, and providing wrap-around and supportive services to prospective clients with arrest, and conviction records. The case manager will help clients identify training and/or employment goals that will potentially help the client obtain and sustain meaningful employment. He/she acts as a coach and trainer for Safer clients enrolled in the Clean Jobs Network Program. The ICM provides effective, comprehensive, and holistic services to each client on his/her assigned caseload. This position works across agency departments and assists in recruiting for training and career pathways related to the Clean Jobs Network Program.

Essential Duties and Responsibilities:

 

Stakeholder Interface and Service:

• Adheres to all federal, state, and local requirements to maintain the confidentiality of clients' personal information

• Maintain engagement once enrolled, to ensure clients remain engaged and are contacted according to their individual needs and program requirements

• Provide counseling aligned with Safer Skills training specifically designed for justice-involved individuals

• Offer available high-demand occupational training to eligible clients, providing support and access to wrap-around services as needed for client success

• Work one-on-one with clients to address issues that arise during programming that threaten clients' suitability for services

• Refer job-ready candidates to the Job Developers, working closely with both parties to ensure job placement success and retention

• Administer contextualized SST (Safer Skills Training) and various assessments such as Work Keys

• Collaborate/refer as needed with other Safer programs to add success of the client well-being and outcomes

• Other duties as assigned

 

Stakeholder Outreach and Recruitment:

• Conduct potential client prescreening and preliminary assessment to determine eligibility/suitability for the program

• Verify clients’ training enrollment, attendance, progress, and milestones with training instructors

• Act as liaison between training instructors and clients to assist in resolving problems as they arise, involving the program manager if needed

• Provide intensive retention follow-up for working clients through weekly contact at 30, 90, 180 and 365 days of employment

 

Tracking, Document, and Reporting:

• Collect required items, documents, and programmatic assessments needed for program enrollment; utilizing items to complete the client intake/registration and enrollment process

• Create, maintain, and update client Audit-Ready hard files following Safer’s Quality Assurance protocol; while documenting all client activities by case notes via electronic database(s)

• Develop Individual Employment Plans with clients documenting barriers and creating plans to accomplish short and long-term goals identified

• Provide reports requested to show program metrics, demographics, services, and other areas related to a specific grant(s) and project(s)

 

Required Knowledge, Skills and Abilities:

• Knowledge of the needs of people with arrest and conviction records, especially reentry populations and initiatives

• Knowledge of human behavior and performance; individual differences in ability, personality, and interests, learning and motivation

• Knowledge of group behavior and dynamics and societal trends and influences

• Demonstrated organizational and problem-solving skills

• Excellent verbal and written skills; able to effectively present information in individual or group settings

• Aptitude for client engagement and intensive case management

• Self-reliant, resourceful, and outcome-driven with the capability to function in a fast-paced, start-up environment

• Hands-on experience and self-sufficiency with Microsoft productivity tools, including Outlook (for email), Office (Word, PowerPoint, Excel), and online collaboration tools (video conferencing, Zoom/Teams, messaging)

 

Education and Experience:

• Bachelors Degree in a related field (Social Work, Human Services, Psychology, etc.), or a minimum of 2 years of experience

• Previous experience in client services or case managing low-skilled low-income adults

• Proficiency with MS Office Suite (Word, Excel, Outlook) applications
