Intensive Case Manager - Walk-In Clinic

A social services organization is seeking an Intensive Case Manager in the Bronx, NY. This role supports survivors of domestic violence and gender-based violence through trauma-informed case management, client advocacy, and coordination of essential services in a walk-in clinic setting.

About the Opportunity:

• Shift: Day shift

• Schedule: Monday-Friday (4 days on site and 1 day remote administrative day; Fridays onsite)

• Hours: 9:00 AM-5:00 PM

• Setting: Hybrid

Responsibilities:

• Provide intensive case management support to walk-in clients in a trauma-informed, client-centered manner.

• Conduct intakes, assess client needs, and develop appropriate service plans and referrals.

• Assist clients with safety planning, crisis intervention, emergency shelter placement, housing guidance, and public benefits advocacy.

• Maintain accurate case notes, digital records, and caseload documentation while safeguarding confidentiality.

• Collaborate with onsite partners and internal team members to coordinate services and support clients with complex and high-risk needs.

Qualifications:

• Associate's degree and 5-10 years of relevant case management experience, preferably supporting survivors of domestic violence or gender-based violence.

• Experience assisting clients with public benefits, housing support, referrals, and related human services needs.

• Strong communication, listening, writing, and documentation skills.

• Demonstrated ability to manage multiple priorities, maintain confidentiality, and work effectively with high-emotion situations.

• Working knowledge of Microsoft Office applications, including Word, Excel, PowerPoint, and Outlook.

• Ability to remain flexible with work schedule based on program needs.

Desired Skills:

• Bachelor's degree in Social Work, Psychology, or a related field.

• Experience working in a Family Justice Center, shelter, or similar trauma-informed service setting.

• Strong relationship-building and cross-agency collaboration skills.

• High emotional intelligence and resilience when supporting clients with complex trauma histories.
