General Summary of Position

 

MedStar Health is looking for an Intensive Outpatient Care Coordinator to join our team at Georgetown University Hospital!

 
The Care Coordinator for IOP (Intensive Outpatient Program) clinical programs functions to support patients in these programs through several roles.  These roles include: coordinating intake procedures into the programs; serving as a co-facilitator in group therapy sessions; coordinating communication with schools, parents, and other support systems in patients' lives; and coordinating discharge planning and post-program follow-up care.  This individual will conduct phone screenings and review completed assessment forms with all interested patients prior to admission to the program.  They will coordinate additional components of care, including facilitating transfer to emergency or inpatient care when clinically indicated, in support to the clinical leadership of the program.  This individual collaborates with all involved teams and partners to ensure that patient and family needs are met.

Primary Duties: 

  • Coordinates care and supports patients/families in overcoming barriers related to insurance, transportation, child care, and other social determinants of health. 

  • Facilitates communication and continuity of care between IOP, external providers, and schools to ensure optimal patient outcomes. 

  • Leads multidisciplinary team meetings and co-facilitates group sessions and program intakes. 

  • Conducts patient screenings, discusses results, and implements follow-up protocols to address clinical and community resource needs. 

  • Maintains thorough documentation and provides regular updates to the care team on patient progress, referrals, and ongoing support needs. 

Qualifications:  

  • Master’s degree in social work. (Required)  OR

  • Master’s degree in Family and Marriage Therapy (Required)  OR

  • Master’s degree in clinical Mental Health Counseling (Required) 

  • 1-2 years Mental Health experience (Required) 

Licenses and Certifications: 

  • LGSW - Licensed Graduate Social Worker (Required) Upon Hire OR

  • LCSW- Licensed Clinical Social Worker (Required) Upon Hire OR

  • LGPC- Licensed Graduate Professional Counselor  (Required) Upon Hire OR

  • MFT - Marriage & Family Therapist License (Required) Upon Hire 

This position has a hiring range of : USD $61,838.00 - USD $111,259.00 /Yr.
