Responsibilities

50% - Direct Patient Care (20 hours/week)

  • Deliver pharmacist-led comprehensive medication management services through direct patient engagement
  • Conduct comprehensive medication reviews, adherence interventions, and clinical consultations
  • Manage complex and high-risk patient cases requiring advanced clinical judgment

25% - Quality Improvement & Clinical Oversight

  • Conduct call quality audits for Pharmacist and PSS teams
  • Provide structured coaching and feedback to improve clinical performance
  • Identify trends, gaps, and opportunities for improvement
  • Support implementation of quality initiatives and performance standards
  • Ensure documentation consistency and alignment with clinical and accreditation expectations

20% - Projects, Workflow Optimization & Cross-Functional Support

  • Lead and support workflow optimization initiatives across pharmacy services
  • Support team members with at-risk scenarios, clinical escalation pathways, and clinical consultation
  • Serve as a clinical education resource for pharmacists and care teams
  • Maintain governance of:
    • Medication Services SharePoint
    • Clinical Guide Hub and internal resources
  • Act as liaison to Clarest Path (CSS) and cross-functional teams
  • Contribute to development and refinement of clinical workflows, tools, and protocols

5% - Onboarding, Training & Team Development

  • Support onboarding and training of new pharmacist hires
  • Serve as a resource for clinical workflows, systems, and best practices
  • Assist with development of training materials and onboarding processes
  • Promote ongoing skill development and clinical excellence
