Position Overview
A large multispecialty group in New York City is actively seeking a Physician to provide high-quality, patient-centered care in nursing home and long-term care settings across NYC boroughs, with a primary focus in Manhattan. This opportunity is ideal for providers who value continuity of care, geriatrics, and building meaningful relationships with patients in a structured, team-based environment.
Key Responsibilities
Patient Care & Evaluation
Provide comprehensive medical care to patients in nursing homes and long-term care facilities
Perform routine rounds, physical examinations, and detailed medical assessments
Diagnose and manage acute and chronic conditions commonly seen in geriatric populations
Prescribe medications and adjust treatment plans based on patient needs
Care Coordination & Collaboration
Work closely with nursing staff, therapists, social workers, and specialists to ensure coordinated care
Communicate regularly with patients’ families and caregivers regarding treatment plans and progress
Coordinate hospital discharges, admissions, and transitions of care
Chronic Disease Management
Manage complex, chronic conditions such as diabetes, hypertension, COPD, and heart disease
Focus on preventive care, reducing hospital readmissions, and improving quality of life
Documentation & Compliance
Maintain accurate, timely documentation in EMR/EHR systems
Ensure compliance with federal, state, and facility-specific regulations and quality standards
Emergency & On-Call Coverage
Respond to urgent medical concerns within the facility setting
Participate in on-call rotation as needed
Qualifications
MD or DO from an accredited medical school
Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
Active or eligible for New York State medical license
Experience in nursing home, long-term care, or geriatrics preferred (new grads with strong interest may be considered)
Strong clinical judgment and ability to work independently
