JOB PURPOSE
As an Internal Medicine Physician at Heartland Community Health Center, you will be responsible for providing comprehensive primary care to adult and geriatric patients. This role focuses on the prevention, diagnosis, and management of complex acute and chronic medical conditions for patients ages 18 and older.
Providers at Heartland practice a holistic and team-based approach to patient care, emphasizing prevention, health promotion, and the management of chronic disease through evidence-based medicine. The Internal Medicine Physician will work within a collaborative, multidisciplinary team that includes Family Medicine, Behavioral Health, Psychiatry, Pharmacy, and Dental Care, ensuring that patients receive coordinated, compassionate, and high-quality care.

Schedule: Full-time, exempt (salaried)
Hours Mon-Friday 8:00AM - 7:00PM (as needed) (average 32 clinic hours/week)
Shifts are typically 8 hours.
Location: Heartland Community Health Center at River City (1312 W.6th Street)
