    Description

    Duties:

    • Work collaboratively with multidisciplinary teams to optimize drug therapy.
    • Provide direct patient care, including treatment assessment and medication management activities.
    • Clinically evaluate all patients in specified area on a daily basis with relevant documentation placed in the medical record regarding pharmacy recommendations, plans, interventions, and medication reconciliation.
    • Review the patient's drug regimen for appropriate indication, dosing, allergy verification, interactions potential, lab monitoring, duplication of therapy, drug interactions and medication documentation.
    • Provide cost-effective management of drug utilization within the clinic.
    • Monitor process and outcome of patient care relating to drug therapy.
    • Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects.

    Requirements

    Internal Medicine Pharmacist Qualifications:

    • Degree/Education: Bachelor's degree in pharmacy. Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education
    • Certifications: Basic Life Support
    • Experience: A minimum of one year pharmacy experience within the last 3 years. Completion of an ASHP accredited PGY-1 Pharmacy Practice Residency or equivalent experience is required. Board certification is preferred. Chief, Department of Pharmacy has option to waive requirement
