• Lead a collaborative care team within an integrated biopsychosocial model
• Manage complex chronic disease populations and build longitudinal, relationship-based care
• Collaborate with on-site behavioral health, radiology, and ultrasound services
• Expand your scope over time joint injections, dermatologic procedures, podiatric care, and more
• Sustainable schedule: MondayFriday, no evening or weekend clinic hours, phone-only call
