Position Overview: The Integrated Substance Use Disorder (I-SUD) Support Specialist collaborates with peer support specialists, behavioral health providers, medical providers, and clients experiencing substance use disorders (SUDs), including opioid use disorder (OUD). This role involves case management, connecting clients to resources, community outreach, scheduling, and facilitating or co-facilitating group sessions. Responsibilities may also include supporting clients in college settings or School-Based Health Centers (SBHCs).

 

What You Will Do:

  • Manage a caseload of clients, providing individualized case management and support services.
  • Conduct crisis intervention for clients in need.
  • Make internal and external referrals for specialty or community services, ensuring client follow-through.
  • Provide education on healthy habits, harm reduction, and SUD prevention.
  • Coordinate and schedule integrated patient visits, including team appointments with behavioral health and medical providers.
  • Deliver Comprehensive Community Support Services (CCSS) for adults with serious behavioral health disorders, including SUDs, and for children or adolescents with Serious Emotional, Neurobiological, or Behavioral Disorders.
  • Conduct SBIRT (Screening, Brief Intervention, and Referral to Treatment) services for clients at risk of or experiencing substance use disorders.
  • Provide psychoeducation, support services, skills-building, and crisis intervention.
  • Participate in community outreach and group facilitation/co-facilitation as assigned.
  • Perform other duties as assigned.

 

Minimum Qualifications:

  • High school diploma or GED.

 

Preferred Qualifications:

  • Certification as a Peer or Family Specialist or Certified Community Health Worker.

 

Physical Requirements:

  • Ability to balance, bend, lift, carry, and pull up to 20 lbs.
  • Ability to sit and stand for extended periods, typically longer than four hours.
  • Flexibility to work a varied schedule, including evenings, weekends, or as needed.
  • Manual dexterity and ability to use hands and fingers for feeling, grasping, and operating office equipment.
  • Adequate hearing ability for communication and monitoring office environments.
  • Good vision for reading documents, operating office equipment, and observing surroundings.
  • Capability to stoop, crouch, crawl, or kneel as necessary for office tasks.
  • Ability to talk clearly and effectively for communication.
  • Ability to walk and move around the office environment.
