Deliver Patient-Centered Care Beyond the Clinic Walls

Bellin Health is seeking a Physician to join our innovative My Home My Care program, serving patients across Escanaba, Iron Mountain, and surrounding communities. This unique role offers the opportunity to provide high-quality, relationship-based care directly in nursing homes, assisted living facilities, and select home settings.

This is an ideal opportunity for a physician who values autonomy, meaningful patient connections, and a proactive approach to care delivery outside of the traditional clinic setting.

Position Highlights

• Full-time (1.0 FTE), Open to part-time
• Mobile, community-based care model
• Provide on-site care in partner facilities and select patient homes
• Strong interdisciplinary team support

Practice Details

• Deliver transitional, acute, and routine care in:

  • Nursing homes
  • Assisted living facilities
  • Select private residences (case-by-case)

• Practice operates as a mobile clinic model, bringing care directly to patients

• Focus on managing chronic conditions and complex patient needs

• Collaborate with a multidisciplinary care team to provide proactive, coordinated care

This model enhances patient outcomes by eliminating barriers to access and ensuring continuity of care in familiar environments.

Compensation & Benefits

• 2-year compensation guarantee

Time Off & CME

• Paid Time Off:
  • 200 hours annually
  • 264 hours annually after guarantee
• CME:
  • $5,000 annual stipend
  • 80 hours of CME time

• Call schedule: 1:12 rotation (2-week blocks)
