Job Requirements

POSITION SUMMARY

The Care Coordinator serves as the day-to-day lead within the Destination Center of Superior Performance and supervise the development, implementation, and organization of the overall program. The Care Coordinator will organize the efforts between surgeons, hospital administration, and staff while tracking the progress of the program and identifying areas that require attention. The Care Coordinator all program management, report generation and review, process improvement, patient satisfaction, facilities review, budget (if required) and administrative activities within the department.

PRIMARY ACCOUNTABILITIES

• Facilitates meetings with hospital administrators, medical directors, physicians, and key staff to ensure alignment and effective communication across all levels.
• Oversees and manages the daily operations and facility management within the Center, ensuring optimal workflow and adherence to established protocols.
• Designs, and synchronizes community education and seminar programs, including responsibility for scheduling, selection of presenters, and preparation of educational materials.
• Coordinates and integrates the program seamlessly within the Orthopedic Service Line to enhance multidisciplinary collaboration and patient care continuity.
• Progresses, and monitors comprehensive quality improvement plans, utilizing program outcomes to drive ongoing process enhancements.
• Collaborates with surgeons to create and standardize care plans and educational materials for pre-operative, post-operative, and discharge phases to ensure consistency and high-quality patient education.
• Prepares and presents key outcomes, metrics, and program updates to Leadership and relevant stakeholders to inform decision-making and strategic planning.
• Aids and collaborates in the development, revision, and maintenance of patient education resources and unit-specific documentation to support clinical and administrative functions.
• Monitor post-acute care by tracking patient progress, analyzing data, and addressing issues to ensure optimal recovery and continuity of care.
• Fosters and maintains effective relationships with patients, families, Associates, and community members to support a collaborative and patient-centered environment.
• Advances the visibility and reputation of the Joint Center by coordinating community lectures, special events, and marketing initiatives.
• Supports driving cost reduction initiatives through engaging the team to decrease readmission rates and length of stay, contributing to efficient resource utilization and improved patient outcomes.

• Knowledge and ability to educate both patients, patient families, and Associates.
• Expertise in achieving results and working with process management.
• Ability to analyze tasks, processes and situations and promote quality improvement
• Exceptional organization skills and demonstrated self-direction.
• Good interpersonal skills.
• Demonstrated ability to lead others in-directly and directly.
• Good oral and written communication skills
• Ability to manage several broad-scoped projects at one time
• Ability to prepare clear and concise documentation within scope
• Ability to work tactfully and effectively with the team members, vendors and community
• Ability to delegate tasks and responsibility.
• Good public speaking skill with demonstrated ability to present to large groups.

PREFERRED QUALIFICATIONS

• Work Experience: Five (5) years' experience in care coordination, project management, or clinical program coordination.
• Licensure: Allied health professional (e.g., RN, PT, PA-C, CRNP).

PHYSICAL REQUIREMENTS

• Majority of time involves sitting or standing; occasional walking, bending, and stooping.
• Long periods of computer time or at workstation.
• Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
• May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
• Communicating with others to exchange information.
• Visual acuity and hand-eye coordination to perform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
