POSITION SUMMARY:The Licensed Clinical Social Worker (LCSW)/Licensed Professional Counselor (LPC)/ Licensed Master Social Worker (LMSW) is a part of the patient care team whose primary purpose is to work in conjunction with the health services within the health center in order to provide assessment, diagnosis and treatment planning of behavioral health to children, adolescents, adults, and families in order to improve psychosocial functioning. RESPONSIBILITIES:• Assists in the development of a system to assess and intervene with high-risk patients• Develops overall resource requirements to meet identified needs• Develops and implements policies, procedures and guidelines• Utilizes current research findings to support clinical social work services best practices on an ongoing basis• Participates in the delivery of care as scheduled• Performs and documents patient's screenings, psychosocial assessments, progress and referral information• Assists in the process of advocacy for patient's access to resources through community programs and coaching to the patients, when appropriate• Communicates treatment plan to patients, families and appropriate staff• Collaborates and coordinates with the caregiver, medical provider, service assessment, case management, home health care and all other disciplines necessary to provide the best care to patients• Provides group and individual counseling to patients on related psychosocial issues• Demonstrates a commitment to service, organization values and professionalism through appropriate conduct and demeanor at all times.• Maintains confidentiality and protects sensitive data at all times.• Adheres to organizational and department specific safety standards and guidelines.• Works collaboratively and support efforts of team members• Will not allow any organizational practice, activity, decision or circumstance which is unlawful, negligent, contrary to the mission, vision or policies of the Mid-Delta Health Systems.

SALARY POSITION:The salary for this position is negotiated between the individual and the Chief Executive Officer within the guidelines of the approved MDHS salary plan. STATUS:This is a FLSA Exempt position.This is an OSHA low risk position.This is an ADA negotiable position.This is a safety sensitive position.Mid-Delta follows state and federal vaccine recommendations.
