Anchor House is hiring a Lead Care Manager to support high-need Medi-Cal members through the CalAIM Enhanced Care Management (ECM) program. This role focuses on overseeing care coordination, supporting care managers, and ensuring quality service delivery.
Responsibilities
- Lead and support care managers in care coordination efforts
- Oversee assessments, care planning, and service coordination under CalAIM ECM
- Collaborate with the ECM Program Manager and external providers
- Monitor quality measures and reporting requirements
- Identify program improvements and communicate updates to the team
Qualifications
- 1+ year of relevant experience required
- High school diploma or GED required
- Familiarity with Medi-Cal, CalAIM, or community services preferred
- Strong communication and organizational skills
- Proficiency with Google Workspace and basic office tools
- Flexible availability, including occasional evenings or weekends
- Multilingual skills a plus
Compensation & Schedule
- $18-$22 per hour
- Full-time, Monday-Friday (on-call/weekend opportunities)
