SUMMARY
The Lead Case Manager has the responsibility to supervise Case Managers and provide oversight to intensive community-based case management and supportive services to individuals and families experiencing homelessness utilizing a trauma-informed, two generation approach. This position will promote long-term housing stability and prevent returns to homelessness by cultivating and maintaining ongoing relationships with property managers/landlords and supporting case management staff in their work alongside participants. The Lead Case Manager will build safe, trusting relationships, and exercise appropriate boundaries and interactions with program participants and ensure training and accountability of case management staff in doing the same. This position will collaborate with the family and multi-disciplinary team to evaluate discharge readiness on an ongoing basis. The Lead Case Manager will apply and coach direct reports to appropriately utilize suggested trauma-informed strategies and interventions in their decision-making and interactions with families and children.
 
RESPONSIBILITIES:
·         Oversee re-determinations of rental assistance eligibility in accordance with Agency policy and Continuum of Care (CoC) procedures. 
·         Ensure tracking mechanisms are immediately updated after a status change occurs by self and direct reports. A status change includes, but is not limited to entry, admission, and exit dates, income and income source at entry and exit. Regularly report performance outcomes in staffing meetings.
·         Demonstrate a deep understanding of trauma and its impact on adults and children as evidenced by the implementation of trauma-informed care principles. Provide day-to-day consultation utilizing Trauma Informed Care principles and a two-generation approach. 
·         Coach and evaluate direct reports on the use of Motivational Interviewing techniques designed to address ambivalence to change and promote engagement and motivation. 
·         Regularly oversee service plan meetings. Assure women and children are treated as the experts and are actively engaged in all aspects of service planning and decision-making directly impacting their lives. Oversee development and implementation of service plans/safety plans and service plan reviews. Review and approve service plans, service plan reviews, and discharge summaries. Provide case consultation and audit files.
·         Facilitate and/or co-facilitate regular Multidisciplinary Team Meetings (MDT) designed to monitor progress; discharge readiness; identify areas of continued need; and recommend self-sufficiency, permanent housing stability, and mental health-focused interventions. Assist program participants with goal setting, needs assessment, information, and referrals. 
·         Supervise case managers for the provision of high-quality, intensive case management services. 
·         Promote and support cross-collaboration with internal and external programs and services to ensure women and their children are referred and linked to need-driven supportive services.

EDUCATION AND EXPERIENCE

·         Master’s degree in social work or related field and at least two (2) years of management experience administering an integrated, holistic model of care through implementation of evidence-based practices.

  • Demonstrate cultural competency through the ability to understand, communicate with and effectively interact with people of various cultural backgrounds. 
  • Attention to detail and the ability to prioritize and execute multiple projects under demanding deadlines.
  • Valid driver’s license and willingness to travel in the community.
