Job Title: Lead Home Care Professional

Location: Independent Living Community & Private Duty Home Care Clients

Schedule: Monday-Friday, 7:00 AM-1:00 PM (On-call 1x/month) or Monday-Friday 4:00 PM - 10:00 PM (On-call 1x/month)

About the Role

We are seeking an experienced and compassionate Lead Home Care Professional to support our private duty home care services both within our Independent Living Community and for clients in the surrounding area. This role is ideal for a seasoned caregiver who enjoys leadership, flexibility, and playing a key role in service growth and care coordination.

Key Responsibilities

• Provide high-quality, hands-on care to clients in the Independent Living Community and private-duty home care clients • Confidently care for a wide range of clients, including those with complex care needs • Serve as a lead and support resource for home care aides, offering guidance and assistance as needed • Fill in for caregiver call-ins to ensure continuity of care • Assist with scheduling needs and coordination of services • Complete introductions and initial visits with new home care clients • Maintain clear, professional communication with the administrative team • Help market and promote home care services within the Independent Living Community • Build strong relationships with clients, families, community staff, and caregivers

Qualifications

Previous experience in home care or healthcare (lead or supervisory experience preferred) Strong ability to work independently while supporting a team Excellent communication, organizational, and problem-solving skills Reliable, adaptable, and team-oriented Comfortable taking initiative and stepping into leadership responsibilities

Schedule & Benefits
