Program/Division Description

Our mental health programs in Solano County provide comprehensive, recovery-oriented services to clients 18 years and older living with moderate to severe mental illness, many of whom also have co-occurring substance use disorders and complex physical health conditions. This position serves both of our Solano County clinics located in Vallejo and Fairfield. Our multidisciplinary team provides psychiatric medical care, case management, therapy, and crisis intervention services in both clinic and community settings, helping clients access housing, primary care, benefits, employment, substance use treatment, and other essential supports. This role is part of a collaborative team dedicated to promoting recovery, stability, and independence through person-centered, trauma-informed care. Community outreach is an integral part of the position, including providing medication support, conducting wellness checks, accompanying clients to medical appointments, and engaging individuals where they live to ensure continuity of care. 

Job Description

The Licensed Vocational Nurse exercises medical responsibilities for psychiatric evaluation and health assessment, medication administration including monitoring for side effects, monitoring for changes in health and mental health status, coordinating care with prescribers, making referrals for additional care, and pharmacy coordination and communication, as part of the treatment team. The LVN assists with crisis interventions, including 5150 evaluations, as needed, and communicates medical issues with primary health care providers. The LVN maintains client caseload and clinical records in accordance with mandated standards, consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care. Additional duties as assigned. This position is based out of our Solano County clinics. The LVN’s schedule will include on-site as well as outreach care, as needed.

Specific Responsibilities

• Exercises medical responsibilities for psychiatric evaluation, medication assessment, and medication monitoring.
• Administers medications and treatments following standard safety protocols.
• Works with an interdisciplinary team that includes clinicians, case managers, and other allied health professionals in the community.
• Ensures proper handling, storage, documentation, and inventory of medications.
• Works in collaboration with the clinical team to provide therapeutic support and psychoeducation to clients, including, but not limited to, harm reduction, relapse prevention, and motivational interviewing.
• Reviews medication refill requests and prior authorization processes according to clinic protocols.
• Participates in quality improvement initiatives and ensures medication room compliance with Felton and Solano County policies.
• Supports with training and onboarding new medical staff.
• Works in collaboration with the clinical team to make appropriate referrals and linkages to other services.
• Coordinates with outside providers to provide more comprehensive care.
• Maintains clinical records in accordance with Medi-Cal standards and Felton policies.
• Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care.
• Optimizes clinical workflows under the supervision of Supervising Nurse Practitioner and Program Director.
• Provides cross-coverage at other clinical sites as assigned to meet patient care and operational needs, adapting to different practice settings while maintaining consistent standards of care and professionalism.
• Performs additional duties as assigned by supervisor or assignee.

Minimum Requirements/Qualification

• Completion of training in an accredited Licensed Vocational Nurse Program.
• Active Licensed Vocational Nurse license from the California Board of Licensed Vocational Nurse.
• Experience working with SMI (severely mentally ill) populations and community mental health settings preferred.
• Demonstrated commitment to the welfare of individuals and families with mental and behavioral health issues
• Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and psychosis. This includes symptoms, treatment alternatives, drug properties and interactions, and preventative healthcare measures.
• Complex problem solving, critical thinking, and social perceptiveness skills, i.e. being aware of others' reactions and understanding why they react as they do.
• Service orientation skills; actively looking for ways to help people.
• Demonstrated interest in ongoing learning and training.
• Ability to be a team player with a welcoming attitude.
• Willingness to travel for outreach to clients in the community.

Additional Information:

Reports to: Supervising Psychiatric Nurse Practitioner

Job Type: Full-Time

Work Schedule: Onsite with community outreach as needed, the schedule is 4 days a week Monday through Thursday totaling a full 37.5 hour work week. 

FLSA Status: Exempt

Salary Range: $105000 - $115000 Annually (DOE)
