Upperline Health is seeking an experienced and compassionate clinic leader to serve as a Lead Patient Care Coordinator. This role blends hands-on patient care with operational leadership to ensure exceptional clinic performance and patient experience.
The Lead Patient Care Coordinator cross-trains as both a Patient Care Coordinator and Medical Assistant while leading daily clinic operations, supporting staff development, and collaborating closely with providers and regional leadership.
This role is ideal for individuals who enjoy ownership, accountability, and mentoring others within a team-based care environment.
The Lead Patient Care Coordinator plays a critical role in sustaining Upperline Health’s patient-centered, value-based care model at the clinic level. This role directly impacts patient satisfaction, staff engagement, workflow efficiency, and overall clinic success.
By leading teams, optimizing operations, and supporting high-quality care delivery, this role helps ensure consistent outcomes for patients and long-term stability for the organization.
