Short Description

The Lead RN Population Care Coordinator (PCC) is responsible for the provision and execution of care coordination and management services for high- and at-risk Cooper Primary Care attributed patients. Additionally, they will provide leadership support for the Population Care Coordination Clinical Care team and program manager. The purpose of the role is to support the sustainability, clinical expansion and optimization of the Population Care Coordination Program in collaboration with the program Manager. It is intended to enhance patient clinical care outcomes, patient satisfaction, provision of quality care, and support the clinical team and program. The role contributes to Cooper as it affords an opportunity to compete with the market as a care coordination and management service, enhance performance improvement, and maintain network integrity.

Maintain responsibilities of Population Care Coordinator Collaborate with PCC program manager and other department managers/ leads as needed Participate in process/quality improvement initiatives to achieve targets as defined by organizational/department goals and objectives.

Support the administrative and clinical needs of the department (e.g. making daily assignments, chart audits, in-basket monitoring and management, workflow optimization, cross-coverage of other Population Health programs) Facilitate new employee orientation and training

Facilitate/coordinate continued education opportunities for clinical team

Lead Population Care Coordinator Department Meetings as designated

Represents Department at departmental, divisional, and organizational level meetings as designated

Experience Required
