Agawam West Rehab & Nursing is seeking a licensed social worker to join our skilled nursing and rehabilitation team.
This leadership-focused role works closely with residents, families, nursing, and rehabilitation staff to support admissions, care planning, and successful discharges.
Compensation
- Licensed MSW: $83,000-$93,000/year
- Licensed BSW: $65,000-$75,000/year
Responsibilities
- Coordinate admissions and discharge planning
- Complete psychosocial assessments, including BIMS and PHQ-9
- Support residents and families throughout their stay
- Participate in care planning meetings
- Collaborate with nursing, therapy, and facility leadership
- Maintain documentation and regulatory compliance
Qualifications
- Active Social Work license required (LSW, LCSW, or LICSW)
- MSW or BSW required
- Skilled nursing, rehabilitation, healthcare, or long-term care experience preferred
- Knowledge of Medicaid and discharge planning preferred
