About the Position
Join our team as an LPN and provide compassionate, person-centered care to adults and seniors in our Adult Day Program. You’ll work closely with participants, families, nurses, and other team members to support their health, independence, and overall well-being.

What You’ll Do

• Monitor participants’ health and wellness
• Assist with health assessments and nursing evaluations
• Administer medications and treatments as ordered
• Help implement and follow individual care plans
• Provide health education and wellness support
• Communicate with nurses, providers, families, and other care team members
• Document care and changes in participants’ health
• Help maintain a safe, supportive, and caring environment

What You Need

• Graduate of an accredited LPN program
• Current New Jersey LPN license
• Current CPR and First Aid certification
• Good communication and organizational skills
• Caring, dependable, and compassionate approach to care
• Bilingual English/Spanish
