Required Qualifications: | - Licensed Addictions Counselor (LAC) or
- Licensed Clinical Addictions Counselor (LCAC)
- One year of addiction counseling experience

Preferred Qualifications: | - Licensed Master Addictions Counselor (LMAC)
- Two years of addiction counseling experience
- Certification by the National Association of Alcohol and Drug Abuse Counselors

Position Summary: | This position provides support, therapy, and case management to individuals struggling with addiction in the Sober Living program. Their primary goal is to assess patient needs, develop personalized treatment plans, and facilitate individual or group therapy to prevent relapse and guide clients toward long-term recovery. |
Essential Job Functions: | - Interview patients, review records, and confer with other professionals to evaluate condition of patients.
- Formulate treatment plan for treatment and rehabilitation of patients.
- Counsel patients individually and in group sessions.
- Counsel family members on how to provide support and the impact of alcohol/drug dependency in their lives.
- Participate in utilization review activities as required.
- Maintain contact on regular basis with referral sources.
- Develop discharge/transition plans and complete discharge summaries.
- Refer patients to other support services as needed.
- Monitor patient’s condition to evaluate success and adjust treatment plans.
- Prepare and maintain reports and case histories.
- Formulate and conduct programs to promote prevention of alcohol and drug abuse.
- Understand and implement all TAP 21 Addiction Counseling Competencies.
- Maintain patient files and keep prepared for site visits.
- Comply with all CKF policies and procedures and related clinical licensure standards.
- Other duties as assigned.
