Job Title: Licensed Clinical Social Worker\n \nImmediate Supervisor Title: Site Leader \n \nOrganization: Goshen Medical Center, Inc. (GMC) \n \nLocation: May vary. \n \nJob Summary and Responsibilities \n \nLicensed Clinical Social Workers (LCSW) at GMC are embedded in the primary care treatment team and are responsible for providing consultative support and assistance to Primary Care Providers (PCP) and for delivering brief interventions for patients regarding behavioral health conditions. The LCSW also assists the team in developing care management processes, such as, the use of evidence-based guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease. Lastly, the LCSW monitors the site's behavioral health program by identifying problems related to patient services and making recommendations for improvement. \n \nPrimary Responsibilities \n \n1. Work as part of the primary care team to support the emotional and behavioral health needs of patients \n \na. Provides PCP consultation. \n \nb. Assists in identifying "at risk" patients and developing patient care plan. \n \nc. Provides brief solution-focused and motivational interviewing interventions to prevent further psychological or physical deterioration. \n \n2. Care Management \n \na. Develops and maintains relationships with community organizations regarding referral needs. \n \nb. Provides information and referrals to patients regarding resources for their basic needs, behavioral health care, and other social services. \n \nc. Transitions patients with chronic behavioral conditions to appropriate community health services. \n \nRequirements and Qualifications \n \nA master's degree in Social Work with a license eligible to practice independently in North Carolina is required. \n \nAt least two years of fully licensed practice experience in a primary care medical setting is strongly preferred. \n \nExperience utilizing motivational interviewing and solution focused models and substance abuse treatment is preferred. \n \nSkills \n \nExcellent working knowledge of behavioral medicine, psychopathology and evidence-based treatments for mental health and chronic medical conditions. \n \nExcellent time management, triage, curbside consultation skills and the ability to formulate succinct, measurable treatment recommendations quickly is essential. \n \nMust be skilled in the fast pace & culture of primary care & working with an interdisciplinary team. \n \nConcise, clear, timely charting in an electronic health record is required. \n \nGood knowledge of psycho-pharmacology (e.g., can name basic psychotropic medications; can discuss common side-effects and common myths; abides by recommendation limits for non-prescribers). \n \nThorough knowledge of the biopsychosocial-spiritual approach. \n \nWorking knowledge of legal requirements for reporting abuse, homicidal threats, involuntary hospitalization, etc. \n \nDemonstrates an ability to establish rapport quickly with a wide range of people. \n \nEffective written and verbal communication skills. \n \nSpanish language fluency preferred, but not required. \n \nTravel Requirements \n \nTravel for continuing education/training/regional meetings as needed (at least yearly). Other occasional travel will also be necessary. \n \nWork Hours \n \nFull-time: 40 hours/week M-F on site during clinic hours is required. \n\n
