Position Summary

Tampa Family Health Centers is seeking a compassionate and clinically skilled Licensed Clinical Social Worker to join our Medical team.

The Licensed Clinical Social Worker provides quality clinical consultation and support for patients experiencing psychosocial concerns while working in collaboration with the integrated primary care team. This role supports both adult and pediatric patients by providing behavioral health interventions, clinical guidance, patient education, and care coordination within a primary care setting.

This position plays an important role in helping patients address behavioral health needs, manage stressors, navigate care, and improve overall wellness through accessible and patient-centered support.

Essential Responsibilities

  • Provide cognitive and behavioral interventions to adult and pediatric patients with behavioral health concerns
  • Provide classes in a format appropriate for a primary care setting, including stress management, sleep enhancement, and chronic pain support
  • Consult with primary care physicians regarding patient care as requested
  • Follow up timely on primary care provider referrals through same-day verbal and/or written communication
  • Complete timely and accurate documentation in the patient's medical record
  • Deliver brief presentations and trainings to primary care team members regarding behavioral health topics and services
  • Participate in the development, implementation, evaluation, and revision of clinical pathway programs
  • Consult with legal authorities as directed
  • Support the Mission, Vision, and Values of Tampa Family Health Centers
  • Effectively manage a full daily patient schedule
  • Provide supervision of peer providers who are in training or not yet fully licensed, as applicable
  • Attend and participate in meetings, committees, and conferences as directed
  • Work collaboratively with primary care team members, patients, and behavioral health professionals
  • Maintain patient confidentiality and follow all applicable HIPAA, accreditation, federal, and state requirements
  • Provide respectful and culturally responsive care to patients from diverse social, cultural, and economic backgrounds
  • Perform additional duties that support the behavioral health department and patient-care experience

Qualifications

  • Master's degree in Social Work or a related field with a minimum of two years of experience as a therapist, or a doctoral degree in Psychology with a minimum of two years of experience as a therapist
  • Current Basic Life Support certification
  • Current and unencumbered license to practice in the State of Florida
  • Licensed Social Worker credential required
  • Bilingual fluency in Spanish is required
  • Two years of experience providing mental health services in a community-based setting preferred
  • Ability to navigate and enter data into an electronic health record
  • Ability to work independently while following TFHC, HIPAA, accreditation, federal, and state requirements
  • Strong written and verbal communication skills
  • Strong organizational and problem-solving skills
  • Ability to work in a fast-paced environment
  • Excellent interpersonal skills and ability to work collaboratively with patients, primary care team members, and behavioral health professionals
  • Ability to work skillfully and tactfully with patients who may be physically or mentally ill, fearful, emotionally distraught, or uncooperative
  • Commitment to maintaining patient privacy, dignity, confidentiality, and safety
  • Ability to provide compassionate care to patients from diverse social, ethnic, and economic backgrounds
