  • Actual offered salary will depend on experience of the applicant\n This is a grant funded position for 15 years\n New Salary Range as of 7/1/25 \n $82,576 - $123,864 annually \n \nSummary of Position: \n \nThe LCSW identifies, triages, and manages patients with medical and behavioral health conditions within Barton Community Health Center and Barton Psychiatry & Mental Health offices. They will provide skill training through psycho-education strategies and will provide brief interventions that are problem-focused and based on goals set in partnership with the patient. The LCSW provides patient-centered behavioral health services, in collaboration with the Patient Centered Medical Home (PCMH), to patients at Barton Community Health Center (BCHC) with support from the medical director and manager. The LCSW applies specialized clinical knowledge and advanced clinical skills in the areas of prevention, assessment and treatment of mental, emotional, behavioral and addictive disorders; engages in multidisciplinary treatment planning and clinical social work interventions. The BCHC LCSW will be part of the substance use disorder care team.\n \nQualifications\n \nEducation:\n\n Masters in Social Work from an accredited institute.\n \nExperience:\n\n Minimum two years of experience as a clinical social worker preferred. \n \nKnowledge/Skills/Abilities:\n\n Ability to perform evidenced based therapies, with little to no supervision.\n Knowledge of community resources.\n Detailed knowledge and experience with evidenced based group and individual therapies.\n Working knowledge of psychotherapy billing and coding.\n Detailed knowledge and experience working with co-occurring populations (chronic health conditions, mental health and substance use).\n General understanding and experience with multidisciplinary teams and PCMH\n Working understanding of Microsoft office and google doc's experience preferred.\n Knowledge of Epic EMR preferred.\n Ability to effectively communicate in English, in compliance with patient safety standards. Bilingual abilities preferred.\n \nCertifications/Licensure:\n\n Current licensure with the Board of Behavioral Sciences as an LCSW in the state of California, dual license in Nevada preferred, in good standing.\n Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.\n \nPhysical Demands\n \nThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.\n\n Routine Hospital/Healthcare & Office/Administrative conditions.\n Contact with patients and guests under a wide variety of circumstances.\n Regularly exposed to the risk of bloodborne diseases.\n Exposure to infections and contagious disease.\n Exposed to hazardous anesthetic agents, body fluids and waste.\n Subject to hazards of flammable and explosive gases.\n Subject to varying and unpredictable situations, including the handling of urgent, emergent or crisis situations.\n Subject to pressure due to irregular hours, frequent interruptions and stressful situations due to multiple demands.\n Occasional travel to various health system locations.\n \nEssential Functions\n\n Provides consistently exceptional care at all times.\n With general direction and little to no supervision provide direct behavioral health services to patients, including:\n Initial mental health evaluations resulting in realistic treatment plans\n Assessing clients using standardized assessment/screening tools, measure, etc\n Provide brief, solution focused therapy\n Provider group psychoeducation/treatment related to chronic medical conditions\n Provide information and referral services\n Crisis assessments/triage with appropriate interventions\n Individual psychotherapy with children, adults, and families\n At a minimum, maintains case load of 5 patients a day with documentation completed within 72 hours of appointment completion.\n Performs evidenced based brief therapies for 12 sessions, from intake to discharge.\n Use of common screening tools and best practices (e.g., PHQ-9, GAD-7, SBIRT, etc.)\n Ability to assess suicidal/homicidal risk and other high-risk situations (psychosis, drug/alcohol use/intoxication), create safety plans and adherence to Welfare and Institutions Code 5150 (W&I 5150), Section 5150 regulations, with little to no supervision.\n Ability to diagnose using the most current Diagnostic and Statistical Manual (DSM) categories and create a treatment plan consistent with diagnosis.\n Collaborative in multi-disciplinary teams and with medical providers to develop individualized treatment plans that integrate medical care and psychosocial issues impacting patients.\n Thorough documentation of services and appropriate billing is completed within a timely manner.\n Uses evidence-based psychosocial treatments for common mental health and substance use disorders.\n Addresses referrals from providers including referrals, providing resources as needed, and providing warm hand-offs as able.\n Attends collaborative behavioral health-based meetings within the hospital and community to enhance cohesive treatment.\n Participates in PCMH committees as needed to assess needs and coordinate services.\n Remains knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.\n* Responds to the needs of the department by performing other duties, as necessary.\n\n
