Position Overview:
We are seeking a Licensed Clinical Social Worker (LCSW) for a hybrid role at our multi-specialty practice in the Bronx, NY. This position requires a minimum of two years of experience and offers the flexibility of working primarily remotely, with one on-site day per week.

Qualifications:
- Master of Social Work (MSW) from an accredited program.
- Active New York State license as a Licensed Clinical Social Worker (LCSW) (required).
- Active Medicaid and Medicare.
- Must reside in the tri-state area.
- Minimum 2 years of clinical experience.
- Proficiency in psychosocial assessments, diagnosis, evidence-based therapeutic interventions, and electronic health records.
- Empathetic, culturally competent, excellent communication skills, strong boundaries, and the ability to work independently and as part of a team.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments and diagnostic evaluations.
- Provide individual, group, and family therapy using evidence-based modalities.
- Develop, implement, and monitor treatment plans.
- Collaborate closely with physicians, nurse practitioners, and other providers within the practice.
- Maintain timely and accurate clinical documentation.
- Provide crisis intervention and safety planning as needed.
