Position Overview:
We are seeking a Licensed Clinical Social Worker (LCSW) for a hybrid role at our multi-specialty practice in the Bronx, NY. This position requires a minimum of two years of experience and offers the flexibility of working primarily remotely, with one on-site day per week.

Qualifications:
Master of Social Work (MSW) from an accredited program.
Active New York State license as a Licensed Clinical Social Worker (LCSW) (required).
Active Medicaid and Medicare.
Must reside in the tri-state area.
Minimum 2 years of clinical experience.
Proficiency in psychosocial assessments, diagnosis, evidence-based therapeutic interventions, and electronic health records.
Empathetic, culturally competent, excellent communication skills, strong boundaries, and the ability to work independently and as part of a team.
Key Responsibilities:
Conduct comprehensive psychosocial assessments and diagnostic evaluations.
Provide individual, group, and family therapy using evidence-based modalities.
Develop, implement, and monitor treatment plans.
Collaborate closely with physicians, nurse practitioners, and other providers within the practice.
Maintain timely and accurate clinical documentation.
Provide crisis intervention and safety planning as needed.
