Licensed Clinical Social Worker (LCSW / LMSW / LMHC / LCAT)
Location: Penn Yan, NY (14527)
Assignment Dates: April 6, 2026 – October 10, 2026 (26-week contract; temp-to-perm potential)
Schedule: Monday – Friday (hours between 8:00 AM – 7:00 PM) + on-call rotation (evenings, weekends, holidays)
Position Overview:
We are seeking a dedicated and compassionate Licensed Clinical Social Worker (or equivalent licensed behavioral health professional) to support a community-based healthcare system in Penn Yan, New York. This role involves providing both crisis intervention services and outpatient counseling across multiple facilities, serving a diverse patient population.
Key Responsibilities:
Provide crisis intervention services for hospital patients at SSMH and GGH
Deliver outpatient counseling services for JDK Outpatient programs
Conduct psychosocial assessments and develop individualized treatment plans
Collaborate with interdisciplinary teams to ensure coordinated patient care
Maintain accurate, timely, and compliant documentation in accordance with NYS OMH standards
Participate in on-call rotations, including evenings, weekends, and holidays
Provide care for patients ages 5 and older
Schedule & Assignment Details:
Monday – Friday schedule with flexible hours between 8:00 AM – 7:00 PM
Participation in on-call coverage (off-shift, weekends, and holidays)
26-week contract with potential for permanent placement
24 non-billable hours
Qualifications:
Master’s Degree in Social Work or related behavioral health field
Active NY license required: LMSW, LCSW, LMHC, or LCAT
Prior experience working in New York State and familiarity with NYS OMH requirements required
Experience providing services to patients ages 5+
Strong crisis intervention and counseling skills
Excellent communication and teamwork abilities
