Licensed Clinical Social Worker (LCSW / LMSW / LMHC / LCAT)
Location: Penn Yan, NY (14527)
Assignment Dates: April 6, 2026 – October 10, 2026 (26-week contract; temp-to-perm potential)
Schedule: Monday – Friday (hours between 8:00 AM – 7:00 PM) + on-call rotation (evenings, weekends, holidays)

Position Overview:
We are seeking a dedicated and compassionate Licensed Clinical Social Worker (or equivalent licensed behavioral health professional) to support a community-based healthcare system in Penn Yan, New York. This role involves providing both crisis intervention services and outpatient counseling across multiple facilities, serving a diverse patient population.

Key Responsibilities:

  • Provide crisis intervention services for hospital patients at SSMH and GGH

  • Deliver outpatient counseling services for JDK Outpatient programs

  • Conduct psychosocial assessments and develop individualized treatment plans

  • Collaborate with interdisciplinary teams to ensure coordinated patient care

  • Maintain accurate, timely, and compliant documentation in accordance with NYS OMH standards

  • Participate in on-call rotations, including evenings, weekends, and holidays

  • Provide care for patients ages 5 and older

Schedule & Assignment Details:

  • Monday – Friday schedule with flexible hours between 8:00 AM – 7:00 PM

  • Participation in on-call coverage (off-shift, weekends, and holidays)

  • 26-week contract with potential for permanent placement

  • 24 non-billable hours

Qualifications:

  • Master’s Degree in Social Work or related behavioral health field

  • Active NY license required: LMSW, LCSW, LMHC, or LCAT

  • Prior experience working in New York State and familiarity with NYS OMH requirements required

  • Experience providing services to patients ages 5+

  • Strong crisis intervention and counseling skills

  • Excellent communication and teamwork abilities
