GENERAL NOTES

The Licensed Clinical Social Worker (LCSW) will play a key role in an interdisciplinary, team-based program, delivering comprehensive care to patients of the Texas Center for Pediatric and Congenital Heart Disease (TCPCHD) at Dell Children’s Medical Center, in partnership with UT Health Austin and Dell Medical School.

A hybrid work schedule (working onsite and remotely) is available for this position after onsite training and successful performance review have been completed.

PURPOSE

The Pediatric Transplant Licensed Clinical Social Worker will provide direct social work services within the Kidney and Cardiac Transplant program, as part of an inter-professional team focused on supporting the psychosocial well-being of patients and families throughout the outpatient portion of the transplant process.  Additionally, the Licensed Clinical Social Worker will collaborate with the team on research, education, and project development aimed at redesigning care for patients in the Cardiac and Kidney Transplant Program, using a holistic and interdisciplinary approach.

RESPONSIBILITIES

  • Biopsychosocial Assessment and Documentation  Conduct specialized psychosocial assessments for each patient in the cardiac and kidney transplant programs to assist the transplant team in evaluating psychological and social readiness for the demands of surgery, recovery, and rehabilitation. Ensure timely and accurate documentation of interactions with patients, families, and service providers in the clinical record, adhering to professional ethics and Federal and State regulations.
  • Clinical Intervention and Treatment Planning Utilize clinical expertise to provide therapeutic recommendations and interventions, collaborating with patients and families to develop individualized treatment plans. Offer consultation, orientation, and training to healthcare staff on the impact of biopsychosocial, cultural, and economic factors on patient care.
  • Care Coordination and Resource Navigation Collaborate in care coordination and complex discharge planning with the interdisciplinary team to ensure seamless service delivery. Assist patients with accessing concrete services, including housing, transportation, and immigration support, to address barriers to care and improve quality of life. Provide referrals for disability services, medical insurance, and other financial resources.
  • Program Collaboration and Quality Improvement Actively participate in transplant team care conferences and selection committee meetings. Engage in professional development within transplant-related and social work organizations (e.g., NASW, STSW). Contribute to the development of program policies, procedures, and social work activities related to transplantation. Support CMS and UNOS survey readiness efforts and participate in transplant quality assurance and performance improvement initiatives.
  • Research, Education, and Interdisciplinary Engagement Participate in research initiatives aimed at improving patient care operations. Engage in the screening, tracking, and reporting of patient outcomes and social work interventions. Actively participate in Health Social Work meetings, continuing education, academic, and research opportunities to promote best practices and interdisciplinary collaboration. Contribute mental health, cultural competency, and social determinants of health expertise to the interdisciplinary team

REQUIRED QUALIFICATIONS

  • Master of Social Work (MSW) from a CSWE-accredited program.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of Texas.

PREFERRED QUALIFICATIONS

  • 3 years of post-degree clinical experience in a healthcare setting.
  • Hold a Certified Clinical Transplant Social Worker (CCTSW) credential and possess transplant experience.
  • Experienced in conducting assessments and delivering evidence-based treatments, with working knowledge of brief interventions, motivational interviewing, and healthcare delivery.
  • Experience working with children, adolescents, and their families.
  • Bilingual in English and Spanish.
  • Basic Life Support (BLS) certification.

SALARY RANGE

$70,000 + depending on qualifications

WORKING ENVIRONMENT/EQUIPMENT

  • Standard office equipment and clinic environment.
  • Repetitive use of a keyboard and computer systems.
  • May be exposed to such occupational hazards as communicable diseases, bloodborne pathogens, ionizing and non-ionizing radiation, hazardous medications, and disoriented or combative patients, or others.
