Job Summary:

Position Title: Social Worker Location: Coxsackie, NY 12051 Duration: 105 weeks Work Schedule: M-F, 7:30am - 4pm
**Pay Rate: $38 -$42/h on W2**
Responsibilities for this position include, but are not limited to:
Provide individual therapeutic services to forensic mental health patients in a residential mental health unit.
Serve as the primary therapist and participate in treatment planning and care coordination.
Conduct comprehensive mental health assessments, including Core Histories and risk assessments.
Complete suicide assessments and ongoing evaluations of patient safety and well-being.
Develop, maintain, and update treatment plans in accordance with OMH policies and procedures.
Prepare accurate and timely clinical documentation, including weekly and monthly progress notes.
Develop discharge plans and coordinate continuity of care services.
Collaborate with multidisciplinary treatment teams to support patient recovery and rehabilitation.
Maintain compliance with New York State Office of Mental Health (OMH) standards, policies, and procedures.
Participate in required orientations, training programs, and facility-specific requirements.
To be considered for this position, you should have: [Skills, Education, or Experience]
Current New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license.
Experience providing mental health counseling, assessment, and case management services.
Knowledge of treatment planning, crisis intervention, suicide risk assessment, and behavioral health documentation.
Strong interpersonal, communication, and clinical interviewing skills.
Ability to work effectively with forensic, justice-involved, or high-acuity mental health populations.
Experience with electronic health records and clinical documentation systems preferred.
Ability to collaborate effectively within a multidisciplinary team environment.
Successful completion of required TB testing and facility onboarding requirements.
Ability to complete CNYPC Orientation and comply with all facility security protocols.
Prior experience in correctional, forensic, psychiatric, or residential mental health settings preferred.
