Duration: 14-Week Contract
Schedule: Monday–Friday | 9:00 AM – 5:00 PM
Pay Rate: $52–$60 per hour

"Navitas Healthcare, LLC" is seeking a Licensed Master Social Worker (LMSW) with hospital-based experience to provide inpatient social work services in an acute care environment. The LMSW will support patients across medical, surgical, and rehabilitation units, delivering psychosocial assessments, coordinating interdisciplinary care, and facilitating safe and effective discharge planning.

• Conduct comprehensive psychosocial assessments for inpatient populations

• Provide therapeutic interventions and emotional support to patients and families

• Collaborate with interdisciplinary healthcare teams to support patient care goals

• Coordinate discharge planning and post-acute services, including community resources

• Advocate for patients’ social, emotional, and environmental needs

• Maintain accurate, timely documentation in the electronic medical record

• Ensure compliance with hospital policies, regulatory standards, and confidentiality requirements

• Minimum 1 year of experience as a Licensed Master Social Worker

• Experience in inpatient medicine, surgery, or rehabilitation settings

• Prior hospital-based social work experience strongly preferred

• Master’s Degree in Social Work (MSW) required

• Active Licensed Master Social Worker (LMSW) license issued by NYSED

• Primary Source Verification required
