Position Type: Full-Time | Part-Time | PRN (As Needed)


Elite Home Health & Hospice is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our interdisciplinary healthcare team. The LMSW provides psychosocial assessments, counseling, emotional support, resource coordination, and patient advocacy to Home Health and Hospice patients and their families.


This position offers the opportunity to make a meaningful difference by helping patients and caregivers navigate healthcare challenges while providing compassionate, patient-centered care.


Position Summary


The Licensed Master Social Worker (LMSW) is responsible for assessing patients' psychosocial needs, developing individualized care plans, coordinating community resources, and collaborating with physicians, nurses, chaplains, and other interdisciplinary team members to improve patients' quality of life.


Essential Job Responsibilities


  • Conduct comprehensive psychosocial assessments of patients and families.

  • Develop individualized plans of care based on patient needs and goals.

  • Provide emotional support, counseling, and crisis intervention services.

  • Assist patients and families with advance care planning and healthcare decisions.

  • Coordinate community resources and referrals.

  • Educate patients and caregivers regarding available services and benefits.

  • Participate in Interdisciplinary Group (IDG) meetings and collaborate with healthcare professionals.

  • Assist families with grief, loss, caregiver stress, and end-of-life concerns.

  • Maintain accurate, timely, and compliant clinical documentation.

  • Advocate for patient rights and promote patient-centered care.

  • Ensure compliance with Medicare, agency policies, and regulatory requirements.

  • Participate in orientation, annual competency evaluations, and continuing education activities.

Qualifications


Required Qualifications


  • Master's Degree in Social Work (MSW) from an accredited college or university.

  • Current Texas Licensed Master Social Worker (LMSW) license in good standing.

  • Minimum one (1) year of experience in healthcare, Home Health, Hospice, hospital, or social services preferred.

  • Excellent communication, organizational, and interpersonal skills.

  • Valid Texas Driver's License.

  • Reliable transportation with current automobile insurance.

  • Ability to travel throughout assigned service areas.

Preferred Qualifications


  • Home Health or Hospice experience preferred.

  • Knowledge of Medicare regulations and Conditions of Participation.

  • Experience with psychosocial assessments and care planning.

  • Experience providing grief counseling and caregiver support.

  • Bilingual candidates are encouraged to apply.
