Requirements:
• Comfortable with regular home visits in Jersey City, NJ
• Valid driver's license and reliable transportation
• Willing to travel locally to meet clients and community partners
• Attend weekly in-person team meetings
• Flexible schedule to accommodate client needs

Qualifications:
• LMSW or LCSW
• Experience with older adults, care management, or related fields
• Knowledge of Medicare, Medicaid, and community services
• Strong interpersonal and advocacy skills
• Proficient in documentation and time management
• Ability to work independently and as part of a team
• Proficiency with generic office software
