Job Title: Licensed Master Social Worker (LMSW) – Medical Social Services

Location: New York, NY 10065
Duration: 27 Weeks (High Potential for Extension)
Schedule: Day Shift – 24 Hours/Week (3 Days)

  • Monday: 9:30 AM – 4:00 PM
  • Tuesday: 9:00 AM – 5:00 PM
  • Wednesday: 10:00 AM – 3:00 PM

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join a dynamic medical social services team for a 27-week assignment. This role supports patients and families in a hospital or outpatient setting, focusing on psychosocial assessments, care coordination, and discharge planning. Experience in transplant services is a plus but not required.


Key Responsibilities

  • Conduct comprehensive psychosocial assessments for patients and families
  • Develop and implement individualized care and discharge plans
  • Provide emotional support, counseling, and crisis intervention
  • Collaborate with multidisciplinary healthcare teams to coordinate patient care
  • Connect patients with community resources and support services
  • Advocate for patient needs and ensure continuity of care
  • Maintain accurate and timely documentation in compliance with regulatory standards
  • Support patients with chronic illness, complex conditions, or transplant-related care
  • Participate in case conferences and interdisciplinary team meetings

Qualifications

  • Master’s Degree in Social Work (MSW) – Required
  • Active Licensed Master Social Worker (LMSW) license in the State of New York (or ability to obtain within one year of hire)Required
  • Prior experience in a hospital or outpatient medical setting – Required
  • Strong knowledge of psychosocial assessments and care planning
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work effectively within a multidisciplinary team
  • Experience in transplant social work – Preferred
