Overview

A growing healthcare organization in Metro Detroit is seeking a Licensed Master Social Worker (LMSW)<\/b> to join an integrated behavioral health team. This role operates within a collaborative care model that embeds behavioral health services directly within primary care, allowing clinicians to work closely with physicians, care managers, and interdisciplinary teams.
<\/p>

This is a structured outpatient -only position<\/b> with no hospital call or inpatient responsibilities<\/b>, allowing providers to focus on delivering high -quality mental health care in a stable clinical environment.

The position offers a hybrid schedule combining in -clinic and telehealth services<\/b>, with manageable patient volume to support quality patient care.

Key Responsibilities
<\/h2>

Outpatient Behavioral Health Services
<\/h3>
  • Conduct comprehensive mental health assessments and psychosocial evaluations.
    <\/p><\/li>

  • Develop and implement individualized treatment plans.
    <\/p><\/li>

  • Provide outpatient therapy services to adults and/or children.
    <\/p><\/li>

  • Deliver crisis intervention and behavioral stabilization when necessary.
    <\/p><\/li>

  • Maintain a structured caseload averaging one patient per hour<\/b> to ensure quality and continuity of care.
    <\/p><\/li><\/ul>

    Integrated Care Collaboration
    <\/h3>
    • Work closely with Primary Care Providers (PCPs)<\/b> within an integrated care model.
      <\/p><\/li>

    • Collaborate with care managers and interdisciplinary clinical teams.
      <\/p><\/li>

    • Assist patients with accessing community resources and behavioral health support services.
      <\/p><\/li><\/ul>

      Documentation & Care Monitoring
      <\/h3>
      • Maintain accurate and timely clinical documentation, including progress notes and treatment plans.
        <\/p><\/li>

      • Monitor patient progress and adjust interventions as needed.
        <\/p><\/li>

      • Participate in discharge planning and prepare case summaries when appropriate.

        Requirements<\/h3>

        Qualifications
        <\/h2>

        Required
        <\/h3>
        • Master’s Degree in Social Work, Counseling, or a related behavioral health field<\/b>.
          <\/p><\/li>

        • Active LMSW license<\/b> in the State of Michigan.
          <\/p><\/li>

        • Minimum 3 years of experience in outpatient behavioral health settings<\/b>.
          <\/p><\/li>

        • Experience conducting assessments, treatment planning, therapy, and crisis intervention.
          <\/p><\/li><\/ul>

          Preferred
          <\/h3>
          • Experience working within integrated behavioral health or collaborative care models<\/b>.
            <\/p><\/li>

          • Comfortable coordinating care alongside primary care and medication -management providers.
            <\/p><\/li>

          • Strong interest in team -based care and working with vulnerable or underserved populations.
