Care IV Home Health is a locally owned and operated home health agency in Fayetteville, AR, seeking a part-time or PRN Licensed Medical Social Worker to join our esteemed team. Requirements:

• Medical Social Worker actively licensed or registered in the state of Arkansas.
• A minimum of three professional references on the application or upon initial contact.

Duties may include, but are not necessarily limited to:

• Provide rehab and supportive casework geared towards restoring patients to their optimal level of social, emotional & health wellness
• Help patients utilize appropriate family & community resources
• Refer patients to appropriate health & welfare agencies
• Consult & coordinate services with physicians and other members of the patient''s care team
• Prepare clinical progress notes & incorporate into clinical record
• Participate in discharge planning & in-service programs
