Licensed Practical Nurse (LPN)

Pay: $37.08 - $40.17/hour

Schedule: 10am - 6pm Monday through Friday

Sign-on Bonus: $5000

Seven Hills Rhode Island is seeking a Licensed Practical Nurse (LPN) to provide compassionate, person-centered nursing care to adults with intellectual and developmental disabilities and complex health needs in residential settings.

As an LPN, you'll help ensure individuals receive safe, high-quality care while supporting their health, independence, and overall well-being. You'll work closely with residential staff, families, physicians, and interdisciplinary teams to coordinate care and promote positive health outcomes.

What You'll Do:

• Provide nursing oversight and monitor individuals' health status, medications, and treatments.

• Support the implementation of individualized health care plans and physician orders.

• Train and provide clinical guidance to direct support and residential staff.

• Monitor changes in health and communicate concerns to RNs, physicians, and the interdisciplinary team.

• Maintain accurate medical documentation, medication records, and nursing notes.

• Participate in interdisciplinary meetings, care coordination, and quality improvement activities.

• Promote infection prevention, medication safety, and compliance with applicable regulations.

• Participate in the Nurse On-Call rotation, including occasional evenings, weekends, and holidays.

• Provide clinical support and guidance to residential programs and assist with day program coverage as needed.

• High school diploma/GED/HiSET required

• Standard driver's license - class C or class D

• Licensed Practical Nurse (LPN) with current license in RI

• 1 - 3 years of related experience
